The ambient AI documentation technology captures visit conversations and generates notes to support provider workflow, clinical consistency, and patient care.
Mentavi Health, a digital mental health care company offering diagnostic evaluations for ages 6 and up and treatment services for adults (18+), today announced the launch of EverCommerce’s (Nasdaq: EVCM) EverHealth Scribe, designed to reduce administrative burden and support more consistent clinical relationships.
Powered by EverHealth and integrated within EverHealth’s DrChrono electronic health record (EHR) platform, EverHealth Scribe transcribes patient visit information and generates structured draft notes for provider review. The tool can help support continuity of care and more informed next-step planning. Clinicians review, edit, and finalize all documentation, and clinical decisions remain with the licensed clinician. Mentavi clinicians can customize their Scribes so their notes remain individualized based on their preferences while maintaining the clinical best practices and Mentavi’s high clinical quality requirements.
As part of Mentavi’s implementation approach, informed consent is obtained both at intake and at every visit. The tool does not train on Mentavi patient data, helping reinforce safeguards around patient data use and privacy.

The launch supports Mentavi’s broader approach to scaling behavioral health care with structured, clinician-led processes that expand access while keeping patient care and documentation consistency central. By assisting with structured note generation, EverHealth Scribe is designed to help protect provider wellbeing, strengthen clinical consistency, and support a more consistent patient experience. Early internal testing also pointed to workflow efficiencies and improvements, including updated patient Problem Lists.
Before launch, Mentavi evaluated the technology through a phased internal rollout with a vanguard group of early adopters, using provider feedback to refine workflow, required documentation elements, and operational fit. During the phased internal evaluation, more than 30 licensed clinicians across two provider waves generated over 14,000 scribe-supported clinical notes (internal data, 2026). An appointment-level analysis conducted with EverHealth compared visits documented with the scribe to visits documented without: scribe-supported appointments showed diagnosis-code capture at approximately 79.7%, compared with 25.5% for non-scribe appointments (internal appointment-level analysis conducted with EverHealth, 2026). This analysis information is a signal that consistent structured documentation is more reliably attached to the visit record when the scribe is in use. Results reflect internal analyses for specific providers during a limited evaluation period and may not be representative of results in other settings. All notes and codes are reviewed and finalized by the licensed clinician.
